"Buford Complex"- Normal variation of labral anatomy.  Rare but normal anatomic  finding where a "cord-like" middle glenohumeral ligament originates directly from the superior labrum at the base of the biceps tendon with an absence of anterosuperior labral tissue.  Misdirected surgical attachment of the cord-like middle glenohumeral ligament to the anterosuperior glenoid results in painful restriction of external rotation and elevation. However, failure to appreciate a pathologically loose superior labrum with this anatomic variant may render the middle glenohumeral ligament incompetent.  Many mistake it for a slap lesion. Check out Buford Complex in a search engine it is very interesting.
